Position Description

Overview
The Director of Social Determinants of Health (SDOH) reports to the VP of Health Homes Operations, with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. Primary Responsibilities
The Director of SDOH will have the following responsibilities:
  • Developing and implementing a strategic plan to address social determinants of health in New York State, in collaboration with various stakeholders, including community organizations, government agencies, healthcare providers, and advocacy groups.
  • Creating and leading initiatives and programs aimed at reducing health disparities and improving health outcomes related to social determinants of health. This may involve overseeing the development of interventions, projects, and policy recommendations.
  • Conducting research, analyzing data, and monitoring health trends related to social determinants of health to inform decision-making and identify areas for intervention and improvement.
  • Collaborating with diverse stakeholders, such as local health departments, community organizations, healthcare providers, and policymakers, to promote cross-sector partnerships and initiatives that address social determinants of health.
  • Developing and advocating for policies that address social determinants of health at the state and local levels. This may involve engaging with policymakers, community leaders, and advocacy groups to promote health equity and address systemic issues.
  • Providing education, training, and technical assistance to various stakeholders on social determinants of health and their impact on health outcomes. This may include developing and delivering presentations, workshops, and training programs.
  • Assessing the impact of interventions and programs aimed at mitigating social determinants of health. This may involve evaluating the effectiveness of interventions, tracking progress, and making recommendations for improvement.
  • Engaging with communities and stakeholders to solicit input, increase awareness, and ensure that initiatives and programs address the specific needs and priorities of diverse populations.

Qualifications
The Director of SDOH should have the following qualifications:
  • Education: Master’s degree in a relevant field such as public health, community health, social work, sociology, or a related discipline, required.
  • Experience:
    • Three to five years of relevant work experience required, particularly in areas such as public health, community health, healthcare administration, health policy, or social services. Previous experience in a leadership or management role, preferably related to addressing social determinants of health, is beneficial.
    • A deep understanding of social determinants of health and health disparities is essential. This includes knowledge of the social, economic, and environmental factors that influence health outcomes and health equity.
    • Experience in policy development and program implementation is required. Candidates should possess the ability to design, implement, and evaluate initiatives addressing social determinants of health.
    • Strong interpersonal and communication skills, including the ability to foster collaboration and partnership with various stakeholders, such as community organizations, healthcare providers, government agencies, and advocacy groups.
    • Proficiency in research methods, data analysis, and the ability to interpret and apply research findings. Familiarity with qualitative and quantitative research methodologies preferred.
    • Experience in leadership, team management, and project management required. The ability to lead, motivate, and supervise staff, as well as the capacity to manage budgets and resources effectively.
    • Familiarity with health-related policies, laws, regulations, and an understanding of how they impact social determinants of health is required. Experience in advocating for policy changes and engaging with policymakers is a plus.
Location: New York, NY
Compensation: $150K-$180K (Based on experience)
